These tools provide information on NH's No Wrong Door (NWD) system and answers to the most frequently asked questions.

Level One Screen

The Level One Screen is intended to determine a person’s potential eligibility for Medicaid-funded long-term community-based services and supports. In addition to Medicaid services and supports, the screening tool will help identify a full spectrum of options based on an individual’s needs and goals. The Screen asks questions that guide the screener to potential referral options, including whether someone is likely eligible for Medicaid-funded community-based services and supports. The Screen is administered to anyone who needs long-term services and supports, including but not limited to, veterans, adults, older adults, children, and individuals with a brain injury, mental illness, or a developmental/intellectual disability.

Long Term Services and Supports (LTSS) Resources and Tools

Medicaid LTSS resources and tools are intended to assist and support NHCarePath Partners and Key Referral Sources when working with individuals who need Medicaid and some Non-Medicaid LTSS.

Companion Guide

The Companion Guide provides information to assist in administering and completing the Level One Screen for both in-person and phone interviews by providing instructions, tools, and resources for the screener to successfully conduct the interview.

Referral Form

The Referral Form is intended to standardize the method NHCarePath partners use to refer clients to other NHCarePath partners. The Form is designed to be the information transfer vehicle and includes information about who-to and who-from, information about the client, and a consent from the client to have his/her information passed along. The vision of the referral form process is to minimize any added burden to NHCarePath partner while improving the client eligibility and enrollment experience.

Core Standardized Assessment (CSA) Process

The Core Standardized Assessment Process intends to ensure that, for each population served, all topics and domains of the Federally defined Core Data Set (CDS) are included. This process aims to allow States to choose the specific questions/items collected within each required topic; the only requirement is that those questions capture the data elements in the CDS.
