Housing/community living involves places to live as well as services to help individuals reside in their homes and communities, even if they need help to stay independent.
Services and Supports for Housing and Community Living
NHCarePath connects you to many types of housing options available throughout New Hampshire. Depending on your needs and preferences, you may choose to live in a private home or apartment with or without services. You may also choose to live in a staffed residence, boarding home, assisted living, or adult foster care where services are provided to you. There are a variety of options that can help you find or keep housing in the community, including:

- Living at home
- Living in independent housing
- Nursing Facilities – find detailed information about Medicare and Medicaid-certified nursing homes.
- Senior housing
- Subsidized Housing / Section 8 – provides safe, decent, sanitary and affordable housing to very low-income households.
