About the Conference:

In 2012, New Hampshire received funding from the Centers for Medicare and Medicaid Services (CMS) to implement the Balancing Incentive Program with the purpose of increasing access to long-term services and supports in community settings.
Conference participants learned more about the Balancing Incentive Program and the important partnerships with community organizations aimed at enhancing access to home and community based supports. Through a series of panel presentations attendees heard about statewide changes to improve access to long term services and supports and efforts to increase consumer awareness regarding community based options as an alternative to institutional care.
Morning Plenary:
Workshop 1: Doorways to Eligibility
Navigating through the eligibility process can be overwhelming and time consuming. This session will include changes that have been made to NHCarePath Learning About Eligibility assure that eligibility determination and enrollment processes proceed in as streamlined and timely a manner as possible. The discussion will include the addition of an “eligibility coordinator” position that will assist individuals throughout the Medicaid application and eligibility system.
Workshop 2: Enhanced Options Counseling for Community Living
Helping individuals and families find the right services at the right time in NHCarePath Options Counseling and Person Centered Planning the right place can be a daunting task even for skilled professionals. This session will define and provide federal best practice standards for options counseling. State programs in options counseling will also be discussed.
Workshop 3: DHHS Client Service Center Enhancements: One-Stop Shopping
This session will unveil the new design for the DHHS Client Service Center and inform participants on how this design will help facilitate a single point of entry or “one- stop shopping” for consumers.
Workshop 4: Demystifying the Home and Community Based Waiver Programs
Medicaid Home and Community Based waiver programs will be reviewed in this session, including a presentation on eligibility requirements, the NHCarePath Waiver Presentation benefits each waiver program could potentially provide to qualifying consumers and the process for applying for each of the waiver programs.
Workshop 5: NHCarePath: Community Resources at Your Fingertips
This session is designed to increase awareness of new and existing tools for finding resources in the community. New informational materials will be shared and plans for a statewide outreach and education campaign will also be discussed.
